After installing the add-on go to Manage Apps and select Feedback & Voting for Jira Service DeskManagement

The list shows all Jira Service Desk Management projects on your Jira instance. Select which projects should have Feedback & Voting for Jira Service Desk Management enabled.
Project configuration
Go to your Jira Service Desk Management project and go to project settings and select Feedback & Voting for Jira Service Desk Management settings
First you need to decide on the Title displayed on portal and Link text in Service Desk portal

This title will be displayed form the customer Feedback and Voting Portal
Link text in Service Desk portal
This text will show as a short cut on your Jira Service
